Insurance provided by Oklahoma Soccer Association is underwritten by Bollinger Insurance. If you wish to file a claim, you must contact the OSA office in Tulsa at 1-800-347-3590 to provide OSA with information for our records. Insurance is provided to OSA members for approved and sanctioned events.
