Online Volunteer Disclosure Form
Process required for volunteer disclosure online form for Risk Management

All Volunteers must complete a Volunteer Disclosure Form (VDF) to participate every year.  All volunteer disclosures are submitted online, so please follow the steps below:

1. Contact your club Registrar and/or Risk Management Coordinator where you volunteer.

2. Provide him/her with your name and email address.  You will be emailed your own secure volunteer login.

3.  Go to www.gotsport.com.  If you are a coach or assistant coach, you will log into your Coach Account.  If you are a Manager, Board Member, or other volunteer, and have a team official profile, you will login through the Manager Account.  (Please note, a Manager account and Team Account are not the same.) 

4. Complete the VDF information using the online "Submit a New Report" process under the "Background Checks" section on the gray menu bar once you have logged into www.gotsport.com.  Make sure to include and verify all of the required information, answer all questions, read the waiver, and then submit your form.

(Please note ALL volunteers must have a volunteer form on file and have a background check to volunteer.  This includes anyone 18 and older that volunteers with a club - admins, board, coaches, managers, trainers, assistant coaches, refs, and other club volunteers.)

Due dates: Fall - September 1st and Spring - March 1st
OSA has developed new Risk Management e-forms to make it easier for all member organizations to submit their seasonal risk managment materials electronically, which are due September 1st.  (Spring materials are due March 1st.) To obtain these forms, click here.

Unanchored Soccer Goals Can Be Dangerous
As part of the Risk Management Policy adopted by OSA, clubs/leagues/associations must complete a Field Safety Inspection Form each season. Part of the OSA form includes portable goal inspection. The article created by Bollinger Insurance Company (see link below) provides information on numerous unanchored goal accidents and how to best prevent accidents of that type from occurring in your complex.
