Oklahoma Soccer Association in conjunction with AIG Insurance Group provides a supplementary insurance policy for adult soccer players registered with OSA and USASA.
The procedure to file for coverage is as follows:
Call the OSA office within 60 days of the incident and provide the office with details of the claim.
The OSA office will mail to the claimant a claim form to complete.
The completed form must be mailed to OSA at PO Box 35174, Tulsa, OK 74153.
The OSA office will forward the claim to AIG insurance once verification of registration is complete.
Following the above actions, all contact between the claimant will be done by AIG.
