Oklahoma Soccer Association in conjunction with K & K Insurance Group provides a supplementary insurance policy for adult soccer players registered with OSA and USASA.

The procedure to file for coverage is as follows:

Call the OSA office within 60 days of the incident and provide the office with details of the claim.

The OSA office will mail to the claimant a claim form to complete.

The completed form must be mailed to OSA at PO Box 35174, Tulsa, OK 74153.

The OSA office will forward the claim to K&K insurance once verification of registration is complete.

 

Following the above actions, all contact between the claimant will be done by K & K.
