Youth Module Level - U6/U8   Fee is $25 

 

 

Youth Module Level - U10/U12  Fee is $25  (Remember to select the course you wish to attend when registering online.)       
E license Course                     Fee is $75 online; $85 by mail
D license Course                     Fee is $165 online; $175 by mail

Use this form to mail your application for any of the above courses.
Complete the form by typing on it, printing it off, and mailing it to OSA, 1235 Sovereign Row, Suite C-11,  OKC, OK 73108  with the appropriate fee. 

Audit: Candidates who wish to audit the course must contact the State Office  email: m.fansher@oksoccer.com
