How to file an insurance claim
Step one - contact OSA at 800-347-3590 or 918-627-2663 and provide information on the claim (CASE REPORT). You will be asked information that a parent or guardian should know concerning the injured player, so have birth date, phone numbers, address, and other information nearby. OSA will submit the Case Report to Bollinger
Step two - after providing OSA the information, go to the
Bollinger web site
and complete the online claim. Bollinger will contact OSA and confirm the Case Report and the registration of the player.
Step three - keep files of your expenses to submit to Bollinger when requested. The policy is a secondary policy with a $500 deductible. Do not have the hospital or medical personnel contact OSA for payment. Claims are paid through Bollinger to the respective medical institution.
