All adult (individuals 18 and over) volunteers with every team, club, and country association must fill out an OSA Volunteer Disclosure form every year and be approved for participation as part of the Risk Management program.  This includes administrators, board members, coaches, managers, trainers, referees, and any other volunteers. 

Please complete this form in its entirety and return it to your CLUB RISK MANAGEMENT COORDINATOR.  Clubs (or Country Associations for their board members) must forward all volunteer forms to OSA in the fall by September 1st or in the spring by March 1st.

1.  Click the following link: Volunteer Disclosure form and print out the form.

2.  Complete the form entirely including social security number, contact info, birthdate, etc. and make sure to answer all questions and sign the form.

3.  Return the form to your Risk Management Coordinator by the seasonal deadline listed above. 

PLEASE NOTE THAT YOU MAY NOT VOLUNTEER UNTIL YOU HAVE A FORM ON FILE AND HAVE COMPLETED A BACKGROUND CHECK, SO IT IS IMPORTANT THAT YOU SUBMIT YOUR VOLUNTEER DISCLOSURE FORM AS SOON AS POSSIBLE.
