Referee Forms

2008 Referee Registration Form - Referees who have lost or did not receive a 2008 registration form from USSF may download a copy here. 

Application To Attend Upgrade Clinic.pdf Referees who wish to attend the Upgrade Clinic held every January must complete this form and either submit it electronically or print it off and mail it to OSA. A check for $25 is required and must be mailed regardless if the referee registers online or by mail.

Referee Assessment Request Form for web.pdf   NEW! This pdf can be typed on, printed, and emailed. Just complete and then hit "Submit." 
 Referees who wish to schedule an assessment must complete this form, save it, and send it to the appropriate Assistant State Director of Assessment. State referees grade 05 must contact the SDA. All other referees requesting an assessment may contact the SDA assistants.

Referee Clinic Contract for web.pdf - Area Referees and/or club officers may use this contract to set up 09, 08, and recert clinics for a club. A $300 deposit is required for an 09 or 08 clinic. No fee is charged for a recert clinic. You may now type on this form and then submit a copy online.

Ref Expense Form calculate for web.pdf - NEW mileage rate! This pdf can be typed on, printed, and emailed. Just complete and then hit "Submit."  Referees and Referee Instructors may find this expense form more useful because it has the codes pre-printed on them and it calculates for you! Updated to include US Mail expenses.

Misconduct Report Form
- New Form! Referees must complete a Misconduct Report Form for all Send Offs of players or coaches. Some leagues require the form to be completed for all Misconducts. You may complete the form online and print it off to mail/fax to the appropriate league. Or, you mail click on the "Submit" button and a copy will be sent to OSA. Please furnish OSA with information as to where the form should be forwarded (ex., GCSA, FCSA, NEOASA, etc.).

Referee Assault/Abuse Form - New Form! Referees who wish to file an assault or abuse complaint must complete this form and submit it to the OSA office within 48 hours of the incident. If a hearing is called for the incident, the entire referee crew is required to attend.You may complete the form online and print it off to mail/fax to the appropriate league. Or, you mail click on the "Submit" button and a copy will be sent to OSA.
 
Assessor Form 2005   - Assessors may download this form for Assessment Report that must be submitted to the SDA. 

Game Log for Referees
Use the Excel spreadsheet to document your games as an official. The Game Log must be submitted to the SRC if you wish to upgrade.
 
Referee of the Year (Adult) - OSA honors its Adult referees with a Referee of the Year award at each Soccer Workshop luncheon. Use this form to nominate a referee 19 years of age or older who is a grade 07 or higher. Deadline - June 30th.

Referee of the Year (Youth) OSA honors its Youth referees with a Youth Referee of the Year award at each Soccer Workshop luncheon. Use this form to nominate a Youth referee 18 years old or younger and is a grade 08 or higher. Referees who turn 19 on August 1 or later are not eligible. Deadline - October 1.

Request to Transfer Registration - If you are moving to another state or moving into Oklahoma and wish to transfer your registration with USSF to Oklahoma, please use this email address link and inform the SRC of your wishes. A form will be emailed to you to complete and submit.

Request to Travel to Out of State Tournament - OSA encourages referees to participate in tournaments outside Oklahoma. The request form will demonstrate to the tournament assignor that the referee is registered with USSF and is in good standing. Additionally, it will include a statement that a Volunteer Disclosure Form and/or background check has been completed. This is an email request. Include your name and the date(s) of the tournament you wish to attend. The form will be returned to you as an attachment.

Reimbursement Application.pdf - Please complete this form as a part of the application process to receive eimbursement to travel to USSF sanctioned tournaments or clinics outside the state of Oklahoma as a duly registered USSF/OSA referee. Complete all information and submit the application to the SRA or the SYRA. 

Based on the criteria and considerations listed below, the Referee Committee will partially reimburse certain expenses incurred by OSA registered referees traveling to USSF sanctioned clinics and tournaments outside of the state. The Committee makes no guarantee that any applicant will receive remuneration in any form. Each application will be considered individually.
Reimbursement for Training, Clinics, or Tournaments – 
Criteria and considerations are as follows:
a.      The applicant must be a currently registered USSF referee, registered through the Oklahoma Soccer Association.
b.      The applicant must have obtained a referee grade of 06 or higher. Grade 07 referees who have stated in writing their intention to upgrade will also be considered.
c.      The applicant must apply for funding two months prior to the scheduled start date of the clinic or tournament. A letter stating the name and location of the clinic or tournament, anticipated total expenses and why the applicant believes this would further his/her referee career must be sent to the SRA or SYRA.
d.      The tournament and/or clinic must be sanctioned by the USSF.
e.      Funding for this program is limited and may not be available yearly. Applicants should not assume that they will be reimbursed for any expenditure unless notified in writing, by the OSA Referee Committee. The applicant will be informed as to the amount of the stipend approved by the Committee.
f.        Funding after the fact will not be considered. All applications must be approved in advance.
g.      If approved, only partial funding will be provided. The Referee Committee believes that individual referees must have a financial stake in their development and advancement.

 

 

 

 Pregame Card - This small card can be printed off and kept in your referee equipment bag to assist referees and assistant referees in their pregame discussions.
