All referees attending upgrade clinics must be file an application and be recertified before attending the class. Application To Attend Upgrade Clinic - this form must be submitted by October 1 with fee of $25
Referee Assessment Request Form for web.pdf NEW! This pdf can be typed on, printed, and emailed. Just complete and then hit "Submit." Referees who wish to schedule an assessment must complete this form, save it, and send it to the appropriate Assistant State Director of Assessment. All assessments must be completed prior to attending the Upgrade Clinic if you wish to achieve the next grade level upon completion of the Upgrade Clinic. Note: Assessment fees are now $55.
