A Region III Interstate Permission to Guest Play form must be completed by all players requesting permission to play with a U.S.Y.S. state association other than the state in which they reside.  It is the responsibility of the player and/or the parent/guardian to submit the Region III form to the appropriate parties within both the home and accepting state associations.

1.      All local club, country (district), and association policies concerning guest play and releases must be adhered to.

2.      If a player resides in Oklahoma and wishes to guest play with another state association team:

          A.)    Complete the Region III Guest form and submit it with proof of registration to the OSA office.

          B.)    An OSA approved Region III Guest form is then forwarded for approval to the state association where the team is registered.

          C.)    The player may not guest play until this form has been approved by both state associations.

3.      If an Oklahoma team wishes to have a player guest play from another state association:

         A.)    The player completes the Region III Guest form and submits it with proof of registration to his/her home state for approval. 
         B.)    The approved guest form from the home state is then forwarded to OSA for approval.
         C.)    The player may not guest play until this form has been approved by both state associations.
4.   Region III Interstate Permission to Guest Play cannot be completed online. It requires a parent signature. The form may be completed online and PRINTED. Then sign and either mail or fax to the OSA office at 918-627-2693.
